Applicants' Information:

Enter the potential Tenant’s information here. The landlord asks for your Social Security number and Driver’s License number in order to run a consumer report/credit check.

If there is more than one applicant, enter their information in the second applicant’s information section. This may include your spouse, partner, roommate etc.….

If any Dependents or minor children will be living with you, enter their name and dates of birth here.

Occupants:

If there are any other occupants who will be residing with the you, the Tenant, enter their names here.

Pets:

Select whether or not you have any pets and enter the pet’s description if applicable.

Applicant’s Employment:

In this section the Tenant will fill out information about their employment history, starting with their current employment. If self-employed, enter such.

Second applicant’s Employment Information:

Enter the Second Applicant’s, if any, employment history information.

Applicant’s Residential History:

Background Check Authorization:

Authorization by the potential tenant for the Landlord to run a consumer report.

Landlord will enter the amount of the nonrefundable application fee to screen the potential tenant.
