Lease Violation (Ohio)

Notice of Lease Violation Instructions:

On the top lines enter the Landlord’s name and address.

On the lines that indicate Tenant’s name and address of premises, fill those out.

On the Notice to line write in the Tenant’s name as it appears on the Lease.

In the first paragraph enter the date that the underlying lease was signed. Then enter the address of the leased premises, make sure to indicate any apartment number or floor number.

In the second paragraph enter the lease provision that the tenant is in violation of and describe the violation. For example, paragraph 5 of the Lease indicates that pets are not allowed and you are currently in possession of a dog that resides at the Premises.

In the next paragraph indicate the number of days the tenant has to remedy the violation or to vacate the premises. The amount of time may be indicated in the lease terms and/or are set by local laws. In the next sentence indicate the remedial action the tenant must take in order to correct the violation. For example, removal of the pet from the Premises.

In the fourth paragraph indicate the time and date that the Tenant must vacate the premises by if they fail to correct the lease violation. For example, by 12 a.m. on September 10, 2019

On the second page enter the state that the premises are located in.

Then enter the date that this notice is signed and sent. Enter the date as day, month and then year.

The landlord should then print their name and address and then sign.

We have also included a proof of service, indicating how the notice was served upon the tenant. This proof of service should be notarized and kept by the landlord as proof that the notice was given.
